The Orient Practice Patient Survey

Your views on the arrangements for your GP Services in the future (for patients registered at the Orient Practice, 75 Oliver Rd, London E10 5LG)

NHS Waltham Forest Clinical Commissioning Group (CCG) have put temporary arrangements in place with the Hurley Partnership since February 2019 to manage GP services at your surgery. The CCG is currently exploring the most suitable arrangements for patient to continue receiving their care from a GP service once the current arrangements expire at the end of October 2019. 

We have engaged with local practices to see whether they are able to register all of the Orient patients when the temporary arrangements come to an end in October, and together with this information, we want to hear your views about what is most important to you and any concerns you might have about re-registering with a local practice or a new provider delivering the GP practice services.

You can complete the survey online or you can complete a paper version of the survey which is available for collection from the Orient Practice reception desk.

Who should complete the survey? 
The questions should be answered by those registered to the Orient Practice, or their carer. If help is needed to complete the survey, please ensure the answers recorded are those of the individual and not those of the person who is helping them.

For each question please tick clearly inside the box that best represents your views, or the views of the patient you are completing this survey for.

You will be asked to indicate on a scale of 1 to 5 different aspects of your doctor’s surgery.

If you don’t know if something is relevant to you then there is a “Don’t Know” option.
